Attention ALL Athletes!!!
The Hemlock High School will be hosting a physical clinic, put on by Covenant hospital, on Tuesday August 12th beginning at 9 am. The cost will be $30 and students will need to sign up for 10 minute slots from 9 am to 12 pm. In addition, you will need to fill out an MHSAA Physical form, as well as Consent to Treat form here: MHSAA Forms prior to arrival.
Cash and checks will be accepted on site. Additionally, you can pay online here: Pay online here. Please make checks payable to Hemlock High School.

Please fill out forms PRIOR to arrival if possible, to help speed things along. All forms will be available on-site for those without a printer.
